DESCRIPTION:Health Action is a partnership of local leaders\, organizations and individuals dedicated to improving health and well-being and reducing disparities across Sonoma County. To achieve its mission\, Health Action focuses on three impact areas: a long and healthy life\, educational attainment\, and a comfortable standard of living. The Gravenstein Chapter is doing listening sessions around the health care district and West County is an important location. We are coming to you to ask you about what is important to you in terms of your health\, including things like economic wellness\, education\, transportation\, etc. We would like to explore strategies to address these priorities with you.
