Gravenstein Community Health Center - Open House

Please join West County Health Centers in celebrating National Health Centers Week at the Gravenstein Community Health Center Open House, with our many friends and supporters on Friday, August 14th from 5-7pm. 

Light refreshments, wonderful company, delightful conversations and tours of our facility will be provided. The program for the evening will begin at 5:30pm and will include many special guests and elected officials.

The Gravenstein Community Health Center is West County Health Centers’ newest and second site in Sebastopol.  This year marks the 50th Anniversary of the National Community Health Center Movement and our theme is “Celebrating our Legacy, Shaping our Future”.

$100 Individual sponsorship for the open house event can be purchased online or you may download the sponsorship form and mail it in. Sponsorships of $100 or more will be posted during the open house and feature the donors recognition name.   

All donations and sponsorships support medical, dental, and mental health care for our low income and uninsured patients at West County Health Centers.
